After completing the assigned readings, consider the following case study and write a paper answering the questions listed at the end of this case study.
Elaine Chang is an infection control nurse at a community hospital. She has been in her position for over 6 years, since finishing her master’s degree in community health nursing. She is a clinical nurse specialist (CNS) one of the four recognized designated advanced practice nurse roles (APN).
She has noted that the annual influenza immunization rate for employees has been decreasing each year after reaching a high in 2009 of 78%. The rate in 2016 to 2017 dipped under 55% for all employees. In addition, this past year, there was a large outbreak of influenza affecting multiple employees and patients.
Ms. Chang has decided she needs to review the situation using Pender’s Health Promotion Model (PHPM) and develop a plan to educate hospital staff/employees and increase immunization rates.
1. Discuss the general purpose, premise, and concepts of middle range nursing theories in general.
2. Give 2 examples of select middle range theories with a brief explanation on how each might be used in nursing practice and research.
3. Provide a brief introductory paragraph which contains a descriiption of the origin of the theory and concepts of Pender’s Health Promotion Model (PHPM).
4. Discuss how Pender’s Health Promotion Model could be specifically applied to the case study. Include the following points in your discussion.
a. Will you consider the data on the immunization rates as valid evidence to create an action plan to promote change? Why or why not?
b. Ms. Chang developed and distributed a survey to each unit manager. After analyzing her results, she identified three major reasons that many in the staff declined the flu vaccine:
A large number (over 40%) believed that the vaccine might give them the flu. They believed this because either they or a family member had become sick with some sort of illness around the time they took a flu shot.
About 25% of those surveyed would have taken the flu vaccine, but it was only available at the hospital Monday to Friday between 10 AM and 2 PM.
About 10% of those surveyed believed that all vaccines are bad for one’s health because they introduce something unnatural into the body.
The remainder had a variety of reasons for declining the vaccine, from religious beliefs to inconvenience. From Pender’s model, what motivational factors might each of these be?
c. What other type of data or information should Ms. Chang obtain for further evidence? This could be possible reasons related to the decline of immunization rates. Explain in a 1-2 sentences why that information is useful.
d. Based on PHPM what actions can Ms. Change implement to to improve the immunization rates? For each action plan provide PHPM concept or concepts that support each intervention.
e. How can she apply interprofessional collaboration in her hospital wide education plan? Who may she include and why?
5. Reflect on how Pender’s Health Promotion Model could be specifically applied to your current nursing practice or work setting.
6. Provide a summary and conclusion about the use of Pender’s Health Promotion Model in advance nursing practice roles.
