– charts/graphs should reflect as followed: 156 admissions total, # of patients 126, 21 patients readmitted within 30 days
Post Implementation: November to May 2024
– charts/graphs should reflect 432 admission total, # of patients 196, 21 patients readmitted within 30 days, 171 patients no readmissions, 145 patients received a 7 days follow up, 49 patients no 7 days follow up, no data for 4 patients
